Step 2: Gather Relevant Data

The next step is to gather data that is relevant to the issue you are looking to address. This initiative considers three main cancer areas of focus. We provide examples of different types of data that are appropriate for breast health equity (BHE), colorectal cancer screening (CRC), and HPV vaccination. You will want to gather the appropriate data based on the focus of your project.

HOSPITAL SYSTEM EHR DATA

The following sections provide guidance on data collection for Breast Cancer Screening and Colorectal Cancer Screening.

BREAST CANCER SCREENING (CMS 125)

Percentage of women 50-74 years of age who had a mammogram to screen for breast cancer in the 27 months prior to the end of the Measurement Period

Denominator: Eligible patients

Definition: Women 51-74 years of age with a visit during the measurement period.

Exclusions: 1) Patients who had a bilateral mastectomy, who have a history of bilateral mastectomy, or for whom there is evidence of a right and a left unilateral mastectomy. 2) Patients who were in hospice care during the measurement period. 3) Exclude patients 66 and older with advanced illness and frailty because it is unlikely that patients will benefit from the services being measured.

Numerator: Eligible patients with appropriate screening

Definition: Women with one or more mammograms during the 27 months prior to the end of the measurement period.

COLORECTAL CANCER SCREENING (CMS 130)

Document the percentage of patients aged 50-75 who had appropriate screening for colorectal cancer.

Denominator: Eligible patients

Definition: Patients aged 50-75 with at least one reportable medical visit within the last 12 months.

Exclusions: 1) Patients with a diagnosis or past history of total colectomy or colorectal cancer. 2) Patients who were in hospice care during the reporting period. 3) Exclude patients 66 and older who are living long-term in an institution for more than 90 consecutive days during the measurement period. 4) Exclude patients 66 and older with advanced illness and frailty because it is unlikely that patients will benefit from the services being measured.

Numerator: Eligible patients with appropriate screening

Definition: Patients with one or more appropriate screening, including 1) Fecal occult blood testing (gFOBT or FIT) during the measurement period; 2) Flexible sigmoidoscopy during the measurement period or the four years prior to the measurement period 3) Colonoscopy during the measurement period or the nine years prior to the measurement period; 4) FIT-DNA during the measurement period or the two years prior to the measurement period; 5) CT Colonography during the measurement period or the four years prior to the measurement period.
