The Hospital Systems Capacity Building Initiative Sustainability Assessment can help you to identify opportunities to improve the sustainability of your initiative. The assessment contains 27 sustainability elements across 10 sustainability categories to help you sustain your chosen evidence-based interventions. The Sustainability Assessment also contains a Resource Guide that lists helpful resources.
