The next step is to create an aim statement that clearly defines the target population, measurable numerical goals, a timeline for achieving the desired goals, and a rationale for doing the intervention now.
EXAMPLE: (Who) Alpha Health and Beta Physician’s Network aim to (actions) increase colorectal cancer screening rate from 57% to 70% for all eligible patients aged 50-75 in select clinics in the Omega County service area by December 31, 2022. This is important because it will (outcome) increase early detection of colorectal cancer, resulting in (benefit to someone) an increase of life-years saved and an overall increase in the health of the community!
