The first step in the process is to create a vision statement with your partners. Developing a vision statement together can be an inspiring and rewarding process. A vision statement will help partners to remain focused on the outcomes and will guide future decisions.
EXAMPLE: By partnering together with a unified message over the project period, we will achieve increased CRC screening rates and decrease late-stage diagnoses that will result in improved health for residents of Omega County!
The next step is to gather data that is relevant to the issue you are looking to address. This initiative considers three cancer areas of focus. We provide examples of different types of data appropriate for breast health equity (BHE), colorectal cancer screening (CRC), and HPV vaccination. You will want to gather the appropriate data based on the focus of your project.
The next step is to analyze the data of focus. The data may provide clear messages, or the lack of key data measures may suggest new data that must be collected.
Ideally, the data will provide enough information to form hypotheses about possible EB/I interventions that can improve measurable outcomes.
The next step is to create an aim statement that clearly defines the target population, measurable numerical goals, a timeline for achieving the desired goals, and a rationale for doing the intervention now.
EXAMPLE: (Who) Alpha Health and Beta Physician’s Network aim to (actions) increase colorectal cancer screening rate from 57% to 70% for all eligible patients aged 50-75 in select clinics in the Omega County service area by December 31, 2022. This is important because it will (outcome) increase early detection of colorectal cancer, resulting in (benefit to someone) an increase of life-years saved and an overall increase in the health of the community!
The next step is to identify possible evidence-based and informed interventions, align them appropriately with focus issues and target audiences, and evaluate and choose the most promising interventions for implementation.
- Example: Interventions to increase community demand: client reminder/recall, client incentives (for colorectal cancer screening), small media (for breast screening and colorectal cancer screening), and patient education
- Example: Interventions to increase provider delivery: provider reminders, provider assessment, and feedback, professional education, provider incentives (for colorectal cancer screening), and standing vaccination orders.
- Example: Interventions to increase community access: reduce barriers and reduce out-of-pocket costs.
The final step in the process is to create an action plan and define activities for your chosen evidence-based and informed interventions.

Evidence-Based and Informed Intervention #1 Professional Education
Intervention Definition. Professional Education. We will compile clinic-level data for a select number of clinic sites to determine those with the lowest screening rates. Then we will educate primary care physicians, physician assistants, and clinic staff at the designated clinics on how to discuss screening options and eligibility with patients.
Intervention Outcome. We will educate 80% of the clinic staff (potential reach: 15 providers and 25 support staff) at the five lowest-performing clinics in Omega County by April 15, 2022. We will track progress by capturing registrations and attendance at the education sessions
